This form is required to join the Frimley Health and Care Online Community Panel. The panel is designed to help us to gather the views of local people, test our assumptions and theories and keep you updated about local health and care services.

Joining the panel will mean you are asked your views through surveys and quick polls sent via the email address you provide. We have many ways to get involved and you can learn about our other projects on our Insight and Involvement Portal
