Your feedback will help us to identify good practices and areas for improvement both of which we will share with our staff.
This questionnaire is anonymous, and we ask that you do not provide any information that may identify you in the comment boxes. Towards the end of the questionnaire, we do include a few diversity questions which will help us better understand health inequalities based on your feedback.
