Are you currently living in Wales, and are a parent or carer of a child or children aged 0-5 years?

 Public Health Wales are looking to improve the information we provide to parents/carers of pre-school aged children around the routine immunisations offered in Wales.

We would like to learn about your opinions and views as parents/carers of children aged 0-5, about any information you may have seen around immunisations, awareness of vaccines, your child's or children's immunisation history, and your experiences of getting your child or children vaccinated.   

We have put together this online survey for parents/carers, which will be shared across Wales to enable us to gather this information.  

This short survey will not ask for any personal information such as name, address or date of birth.

Reports prepared using answers from this survey will not contain any personal information and will protect your anonymity. We may quote feedback that you provide, but we will make sure that it does not identify you in any way.

There is also an option to withdraw the answers to the survey after completing it using a unique reference number, if you change your mind and no longer wish to participate. 

The survey will help us to develop better and more appropriate information resources, as well as supporting improvements locally, Public Health Wales would really value your opinions as a parent/carer to achieve this. 

The survey closes on 30th November 2025
