We are inviting you to take part in a survey because you have recently use the Cancer Care Navigator service. We want to know what you think works well and what could be improved.
The NHS needs patient feedback if it is to improve and develop the services it provides. The questionnaire is designed to capture the minimum amount of information that we believe is necessary to help us do this.
You do not have to take part in this survey if you do not wish to do so. If you choose to not take part, this will not affect the care and treatment provided. This questionnaire is anonymous, and you cannot be identified by the information included.
The feedback from all participants will be analysed, and a report will be provided to the Cancer Care Navigator Service to support them in reviewing and improving their services where required.
