Wellbeing Plan - Service Users and Carers Survey

The Trust is committed to providing excellent health and social care for both people with mental ill health and those with a learning disability. The Wellbeing Plan is currently being reviewed and improved and we would like to hear your views on the current Wellbeing Plan and also what is important to you in planning your care and support and developing a plan.


The Wellbeing Plan is the document that records the plan for your care, treatment and support.  This can be a short term plan over a few weeks or a longer term plan over a year. 


The Wellbeing Plan should identify your personal goals and aspirations and what you and other people (staff, carers, other agencies) will do to support you in taking steps towards achieving these. The Wellbeing Plan will also identify what helps you when things get more difficult, this is sometimes referred to as a crisis plan.


Alternative Care Plan are used for people with dementia or learning disabilities so this survey will not be relevant to you or the person you care for if your needs fall into any of these groups.
