In order to improve the service we provide, we are working collaboratively with the National Palliative and End of Life Care Programme carrying out a survey of patients who are currently receiving care and support from the Palliative Care Team within our Organisation.
The survey should take around 3 minutes to complete, is voluntary and you can choose not to answer some or any of the questions. Your answers are anonymous, and we will use, manage and sort all data in accordance with the General Data Protection Regulation (GDPR) 2018 currently in use in the UK. No personal identifiable data is being collected for the purposes of this survey.
When answering the questions, please help us by giving your honest opinion and please do not include anything personal that you do not wish being made public, as on occasion we might wish to share your anonymised feedback.
We do not need to know your personal details, but we will ask some optional general questions about who you are, later in the survey. This is so that we can make sure we are asking all groups of people about their experience.
