Moving from Children's to Adult Services

This survey is for teenagers and young adults with a long term health condition, who have recently moved, or are due to move (transition) from Children's to Adult services.

If you are a child with a long term health condition, we aim to start preparing you to move from Children’s to Adult services at Gloucestershire Hospitals from the age of 14.

Most young people will change services between 16 and 18, but we want to help you get ready as early as possible.

Our goal is to make sure you feel safe, supported, and confident about this change.

If you are aged 14-25, either moving towards your transition to adult services or an adult who has recently made this transition, we want to hear from you. We also welcome feedback from Parents and carers.

This survey will help us understand what matters to you.

Your answers are anonymous - Please do not include any identifiable information, including names or contact details.

Please be honest about any worries or challenges. Your feedback will help us improve how we support young people.
