In order to improve the service we provide, Cardiff and Vale University Health Board would be grateful if you could complete this short questionnaire regarding our services for babies, children, young people and their families. When answering the questions, please tell us what is most important to you and how we could improve these services.

This questionnaire is voluntary, all the results are anonymous and it should take around 3 minutes to complete. When answering the questions, please help us by giving your honest opinion and please do not include anything personal that you do not wish being made public, as on occasion we might wish to share your anonymised feedback. We do not need to know your personal details but we will ask some general questions about who you are, later in the questionnaire. This is so that we can make sure we are asking all groups of people for their feedback.
