Health and social care organisations in Blackpool are working together to improve services for adults who have palliative care needs or are in the last year of their life.
The organisations, which include Trinity Hospice and Blackpool and Teaching Hospitals, would like as many people as possible who are using these services to complete a short, anonymous survey about their experiences.
They are also keen to learn about the experience of family members and carers of people who are in receipt of palliative and end of life care services.
